Community Care Planning 2010 - 2013
Clackmannanshire Council Adult Care and Housing Services work in partnership with NHS Forth Valley through the Clackmannanshire Community Health Partnership to plan, provide and develop support and services to people who have community care needs in Clackmannanshire.
People who have community care needs might be
- older people (aged 65+ years),
- people with dementia,
- people who have mental health problems,
- people who have physical disabilities,
- people who have sensory impairments,
- people who have learning disabilities,
- people who need social work support or services for other reasons, for example illness, problems arising from substance misuse, or homelessness.
Joint service planning also takes into account the needs of Carers.
Service planning
Service developments and future planning are aiming for better access to services and improved outcomes for service users and their carers.
In each area of service planning there are reference points and links to more detailed health, housing and social services plans which are in place.
Consultation with service users and their carers is also an important aspect of service delivery and planning.
Working towards national Community Care Outcomes we are aiming for
- service user and carer satisfaction with services provided,
- faster access to services,
- better support for carers,
- improved quality of assessments and care planning,
- reduced risk of admission to hospital and greater independence,
- development of more joined-up community care and health services.
This is a time of considerable change and development in the way services are provided in partnership with other organisations, examples of which are:
- Community Health Partnership
- Carers Strategy
- Change Plan
- In 2010 the Scottish Government launched a new strategy 'Re-shaping Care for Older People' that requires local authorities and the NHS to work in partnership with other agencies to improve local health, housing, social care and community support services for older people.
- Change Plans are being developed for implementation over the next five years.
The Clackmannanshire Community Health Partnership and Social Services Community Care Plans for 2010 - 2013 will be posted onto this page when they have been revised.
