Community Care Planning 2010 - 2013

Clackmannanshire Council Adult Care and Housing Services work in partnership with NHS Forth Valley through the Clackmannanshire Community Health Partnership to plan, provide and develop support and services to people who have community care needs in Clackmannanshire.
