Community Care Assessment Teams

Clackmannanshire Council Social Services has three Community Care Assessment Teams, one of which is an Intake/Duty team, operating within the Clackmannanshire area. The teams consist of
- Occupational Therapists;
- Social Workers;
- Mental Health Officers;
- Social Care Officers.
The teams can provide an assessment, i.e. an evaluation of your community care needs, or problems you may be facing, which may help you access services such as:
- Home Care
- Meals-on-Wheels
- Community Alarm Service
- Equipment and Adaptations for People with a Disability
- Respite Care
- Day Care
- Residential Care
- Nursing Care
Community Care Charging Policy
There may be a charge for the services provided:
- Many services are free to the public;
- Some services are charged on the person's ability to pay, for example:
- major adaptations;
- homecare;
- residential/nursing home care.
- Other services such as meals-on-wheels and respite care have a fixed charge.
