Community Mental Health Team (CMHT)
The Community Mental Health Team functions as a specialist multi-disciplinary team who provide assessment and treatment for adults with severe and complex mental illness.

The philosophy of care is that the team aim to assist people in their own home environment, where possible, whilst helping them to tackle their mental health care needs. The focus of care is individualised and personal to the identified needs of each client.
What do we provide?
Getting started
If you are referred to the Community Mental Health Team you will initially be invited to meet with one or 2 members of our team of mental health professionals. This meeting may take place either at your home or at the Mental Health Resource Centre depending on your particular needs and circumstances.
At this initial appointment you will have the opportunity to discuss any issues relating to your mental health and wellbeing. In collaboration with you and our team we will plan the best way forward according to your identified needs.
What we offer
The Community Mental Health Team offer a variety of interventions to assist people in the development of positive coping skills, symptom management, relapse prevention as well as clinical support for people who have mental health care needs requiring longer term assistance.
Our team is made up of Psychiatrists, Junior doctors, Community Psychiatric Nurses, Psychologists, Social Workers, Occupational and Art Therapists.
Each person who is referred will have a named worker and care plan which is suitable to their own identified health care needs.
The CMHT also work to actively promote positive mental health and wellbeing and to encourage service user and carer participation in care plans where requested.
The length of time that people attend the service will vary according to their individual needs and may range from a few sessions to several weeks or months. The team will work with you closely and as a partner in your care to make sure that the service offered to you is at the right time for the right reason and in the right place. Any move on from services when things have improved for you will also be planned so that the transition is as smooth as possible.
The CMHT is actively involved in local and Forth Valley wide forums to continue the development and improvement of mental health services for the benefit of service users and carers.
How to access the service
If you feel you would benefit from our service, please contact your GP, Social Worker or other professional you may be in contact with. They will refer you via the Single Referral Pathway, and we will aim to contact you within 6 weeks of receiving the referral.
