Application for National Entitlement Card (60 years & over)
You must bring with you 1 document to confirm proof of age and 1 document to confirm proof of address (dated within the last 3 months). See examples below:
Proof of Age
- Birth Certificate
- Passport
- Photo Driving Licence
- Attendance Allowance Award Letter
Proof of Address
- Council Tax Bill
- Utilities Bill
- Bank Statement
- Television Licence
- Letter from Residential Home
An application will be completed by one of our staff, signed by you and you will also get your photograph taken. The completed application will then be sent away and your National Entitlement Card will be sent to you through the post within approximately 14 days.
